As far as testing the file for modifications, what about doing an _gs_size()
call before each access. If _gs_size != old_size, then rescan. Obvious gotcha
is if you didn't change size, but altered the file (corrected a typo  etc.).
Another option might be to flag the time at file open, and then check the last
modified time before each access. This may or may not work, as I don't remember
if the last modified time is updated before the file is closed.
