At least in the Chicago area, you most certainly ARE charged for ambulance
rides. My one ride in an ambulance resulted in a $200 charge. A number of
State of Illinois retirees complained that they were charged by the City of
Chicago and other communities also.

I will note that a friend who was an EMT told me that the fire department
he worked for didn't charge for the first call within a calendar year, but
charged $150 after that.
