I lost a good friend to oesophageal cancer ten years ago.  They (doctor and
MacMillan nurses) kept him awake and lucid to the day before the end.  Then
he asked for enough to ensure he wouldn't feel pain again.  The doctor's
response was that he couldn't do that, but he was going to be busy during
the week so he'd leave a week's supply.
