This is a problem with the way Microsoft believes Headings and 
Numbering should be related. Unfortunately most of the rest of 
the world (as far as I can tell) doesn't... :-( In my view, this 
has been an architectural anomaly in Word since v1!

You can't get the the exact layout you showed using Heading 
numbering. You can do it using AUTONUMLGL fields in the headings 
- then just add the word "Chapter" to the Heading 1 style...
