Maybe you have a phone line which isn't too tolerant. Noisy phone lines
will make it hard to sustain higher speed connects.

Add X3 to your dial string (ie ATX3DT). This will force the modem to
blindly dial out rather than attempt to detect the service tone, which it
evidently doesn't seem to be capable of.
