The alternative to mandated coverage is to leave the business. When *all* health insurors leave the business one's choice will be the government. Presumably that same government will also outlaw the use of cash, checks or credit cards in payment for medical services - to prevent those evil rich people from acquiring medical services they aren't "entitled" to. Note, however, that those evil rich people will simply go to someplace outside the control of the government to obtain the services they want. They may even permanently relocate outside the reach of the US government, and take as many of their assets with them as they can.
