But, as to the grammar, "Mr Bush" would be correct in most every other English-speaking country. I'm not quite sure where the "Mr President" came from, but it appears to be a way of both negating the high office and recognizing it at the same time. In any event, I have never heard someone talking or writing about the president using "Mr President so-and-so". It has been "President ".
