When I used to trail ride, I put the halter OVER the bridle, and it seemed to 
be a little more comfortable for the horse and didn't interfere with signals. 
 Regarding the halter/bridle combo, yes they are available.  If you have a 
problem finding them in your area, try a catalog that carries Australian 
saddles and related tack.  They're fairly common with the Aussie-style 
rigging and make tying up to take lunch or rest stops much easier.  Not to 
mention that if you have problems like an injury to the horse, a halter is 
much sturdier, usually, then a bridle and you don't have the potential of 
injury from the bit.
