Easy. Use the time as the base for the counter, so the least-significant
bits are an offset from the time. Of course, this still implies a limit on
the number of unique IDs that can be generated per second, but at least the
limit can be set a lot higher than 1/2. :-)

... Maintenance Free:  It's impossible to fix.
