Noo ... twas just a little flag - and the loos were much bigger in 
those days...

Besides which, us British used not to scream much - a discreet 
cough or gasp at best, I fear - and in extreme cases, apologetic 
as in "Sorry to bother you, doctor, but a few days ago I seemed to 
have been silly enough to encounter a pointy object and I wondered 
if you might spare a momen...."
