Make sure the shortcut is configured to run in the Blue Wave folder.
I would have similar problems with Win 3.11 if the PIF file weren't
configured with a "Start-up Directory"
Make sure there is an entire path listed to the BWPACK.BAT file.
Edit the file, so that each command line used has an entire path
listed:
