It's supposed to be an anonymous ballot.  I have no problem providing ID at
the polls, though, and I've done it even though I was specifically told I didn't
have to.  IMO as long as there's no way to connect the individuals on the rolls
with the votes cast there's no problem.

It depends.  If you're not operating a motor vehicle you don't.

Maybe.  Some jurisdictions don't require it.

from a licensed dealer.
