I have a patient I'd like a bit of help with:
He's an 81 y/o Colonel who is only comfortable within an
extremely narrow temperature band of about 79-80 degrees F. He has
to wear several layers of clothing so he can add or subtract as
the need demands, and this is gradually ruining his social life
outside his home.
Full neuro exam, blood work, thyroid check, etc are all entirely
normal. He is slight of frame and slender. Serial oral temps are
all normal and in a close range, no matter if he feels hot or
chilled.
I can come up with three possible causes:
1) a small lacunar infarct somewhere in the hypothalamus
2) "Male Menopause"
3) Obsessive-Compulsive D/O or some other "functional"
diagnosis.
Anyone have any thoughts? He's quite demanding, and I've done all
I can think of (serum testosterone levels are pending)
