I'm assuming it's an internal (haven't got the original message available on 
this system), but there should be some identification on the board itself.  
On the USR 14.4 Sportster we pulled out of this particular system sometime 
back, the id's are on the back of the board... USRobotics (r) Copyright 1994 
1.012.0266-C  Model 0266 Along with 'Product Rev' which was blank and A logo 
of SAC (r) LR64979 Information Technology Equipment (UL) listed 70N3 Made in 
the USA...
Along the backplate of the board, there are FCC ID and registration numbers
FCC I.D.:CJE-0266
FCC REG. NO.:CJEUSA-65828-FA-E
COMPLIES with PART 68
FCC RULES RINGER
E0: 0.28
D.O.C. LOAD NUMBER:20
I'm sure there are some similar sets of numbers on your modem, especially as 
far as the required FCC numbers.
There is a database on www.fcc.org which for locating those
particular numbers and identifying them as to maker/model.
