Is the fallback based on both time and error-rate i.e. if X errors/sec
for 2 secs causes a fallback, will 2X errors/sec cause a fallback after
1 or 2 secs?

That's proving to be a bit of a pain while I'm researching efficiency
figures. I connect at 31200/31200, find later on it's up to 33600/31200
and then latter still its back to 31200/31200. Pity the USR V.Everthing
has no front screen display of recieve/send speeds.
Using LAP-M with 244/15 on a 900K zipfile at 31200 and 33600 yesterday I
saw: 114.7% - Zmodem with 32-bit CRC and 1K subpacket size; 118.2% -
YMODEM-g. I downloaded the same file 6 times and kept using +++ ... ATI6
to check connection speeds. So I'm reasonally sure that these figures
are correct.
