That's a more complex issue as it evolved overtime (2009-12) and I have
seen sites with a number of variations based on the beta testing/discussions.

The basics of it were:
 
  \http\public\AccountCreated.wct
  \http\public\newuser.wct

  \http\template\SignUpPage.css
  \http\template\SignupPage.wct

  Optional:

  \http\template\Signup-Header.wct
  \http\template\Signup-Footer.wct

  Apps:

  html-captcha,wcx
  html-checkcaptcha.wcx
  html-wcajax.wcx


There are also associated error templates and cookie checks.
