Trek IV: The Voyage Home.  The second was Capt. Tryla Scott, the youngest 
Starfleet Captain ever, in TNG's first season episode Conspiracy.  Capt. 
Pillipa Louvois,
who appeared in the 2nd season episode The Measure Of A Man was a JAG 
fficer,
not a Starship commander, so she doesn't count.
