What's worse is keeping tens, or hundreds, of geographically dispersed
servers patched.  That pretty much precludes your "click to apply
one" method.

I don't think MS is too different from Linux in this regard.  They're big
enough and there's enough intracompany competition  that OS and IIS patches
might as well be coming from different places. While you are downloading
patches for both products from the same place, there is no guarantee that
there was any real testing of the interactions done.
