Picture a virus that besides loading a bot on your machine also edits the
system so it now points to the hackers update server instead of microsofts'
update server. Ok now the machine gets cleaned of the bot, but come the
second tuesday of next month it informs the user of a new patch and the
patch it downloads marks the system as patched but instead of patching (or
in addition to patching) also reinstalls that month's bot version again.

How would the user know how he got infected again? Every time he got
cleaned he would check to make sure he's patched and get reinfected by
checking.
