From: "John Beamish" More likely they were marks made by the proofreader examining the galley proofs. One underline shows the text to affected, "ital" in the margin says "Italics". Two underlines shows the text and "SC" (or was it "small" -- can't remember) is small caps. Three lines and caps is, as expected, all capitals. Done with a blue pencil (I don't know why). I remember my dad doing that on a number of occasions.
