AHA! You just told me something that gives me the key to it all! :) Though I
can't resolve the problem, I *can* tell you *why* it's doing this!

See, Blue Wave (the .exe) is a DOS program, made for MS-DOS which does not 
use icons. Windows also uses .exe but those .exe programs *include* an icon
*inside* the .exe. DOS .exe programs do not. You can't tell the difference
by the filename. The only way you'd know is if you already know it's a DOS
program and/or if it shows up in Windows Explorer with a default icon by the
filename (if you are viewing in icon, thumbnail or tile mode). So the reason
it shows a ? in the Office toolbar is because the .exe was made for DOS and 
thus does not include an icon in it. The shortcuts (.lnk or .pif) are not exe
programs but just *links* to the program, which include a *pointer* to the
icon. These also do not *include* the icon in the link (.lnk or .pif) file. You
might not even have seen the .lnk or .pif file extension. But it's the same as
the "Shortcut to..." file you click on when you've made a
shortcut. So these
also may show up as a ? in the Office toolbar.

I think you might be right that if you can not make a shortcut in the Office
toolbar then it's not possible to get the BW icon to show up.

Try *right Clicking* on the Office toolbar though and look for something called
"Customize" or "Properties" or "Settings",
maybe "Configuration"? Click on that
if one of them is present and see if you can add a shortcut to the toolbar 
that way. Might be worth it to explore those options and look around. Or even
use Google and see if there's tutorials on adding a shortcut to the Office
toolbar without dragging it from the desktop or other places.
