Hi Richard,
if you'll download the most actual shareware version of Telix for DOS
(v3.51), you'll find a file called AUSSIE.REG in your Telix directory
after installing successfully which contains the reg form for Australia.
Alternatively, pressing Alt-3 while Telix is running will print a reg form
on your connected printer. Moreover, you should also be able to
registering Telix online by pressing Alt-4.
BTW, the official Telix reg site for Australia is:
My Computer Company
PO Box 99
Erskineville, NSW 2043
Phone: +61 (02) 565-1991
  Fax: +61 (02) 550-4459
best regards,
Bernd/2
