The name of the unit is ASAP RD4000 Ring Decipher, manufactured by Command 
Communications, Inc. in Aurora Colorado.  I have been told they are only sold 
thru Graybar Electrical Supply, a nationwide chain.  Check you local yellow 
pages - there might be one somewhere close to you.
