Actually, I've filled out such a questionaire that was issued for a
student long since diagnosed. The physician was trying to monitor the
medication dose of Ritalin. As I recall, not only did I answer questions
about academic performance (such as "understands and can follow written
directions without confusion") but also behavior questions about
concentration, attention span, etc...
