The simple answer is that you can't.  The partition table only allows for 4
entries.  You have three primary and 1 extended partition recorded in that
table.  You may have to look into another solution such as System Commander
which does not require a partition for itself.  LILO might also work but I
have played that much with LILO since my Linux box is only used for Linux and
a DOS emergency boot at the moment.
