Back in the DC-4 days, maybe also on the DC-6/7,  windmill starts were made 
on occasion in a remote spot where parts were scarse.  Not with passengers on 
board, only the crew.  Then taxi back and load the passengers with the engine 
in idle.
AAL tried to make a three engine takeoff here at Denver one time in a B-707 
and they ended up off the runway.  They gave power to the three engines 
rather than symetrical power until after VMC.  If I recall it was a 
Management Crew.
