I would suggest Category 3 UTP (Unshielded Twisted Pair).  I just had some 
new phone lines put in, and thats what they used to bring it into the house.  
Cat 3 should be more than adequate for phone/modem use.  Cat 5 would be 
overkill unless you plan to run 100meg Ethernet on the same cabling plant 
some day...
