Hello Robby I just wanted to drop in and say that HOLE-In-The-Head is 
almost always caused by a Dirty Tank.(Sorry to say) Once A fish 
develop es this there is NO WAY to get rid of the marks left behind. 
Most people feel they change their filters often enough, change enough 
water, and so on.  My experience has shown me that most people do not 
use a gravel washer when changing water, and do not change enough water 
during water changes.  I have kept very large mini-reef systems, very 
large cichlids tanks and they all had one thing in common, 1/3 water 
changes weekly with a gravel washer.  Cleaning filters, and filter media 
do not take the place of a good water change. Due to my prat ices I have 
kept some very delicate species, and have never used any chemicals or 
medicines in any way, and have had no problems breeding fish in anyway. 
Another way you can check your tank to see how clean it is, is to stir 
up the gravel and look for a very small clear worm like creatures about 
a 1/4 inch long.  These are called WHITE WORMS and are another sign of a 
dirty tank.  These are also eliminated by water changes with a gravel 
washer.  So the bottom line is develop good maintenance practices, and 
water changes.  The fish will love you for it...
