  Generally on the part of the insured. Health care in the US is largely 
other people's money. Less than 20% of all health care expenditures in 
the US are out of the pocket of the person actually using the services 
(and that includes the OOP part of the premium). When you are playing 
with OPM (Other people's money) you demand (use) more and the price goes 
up (especially in health care where the barriers to entry are so high.. 
You can't just decide to "build" a new doc tomorrow). 
   While some of OPM problem is seen in other countries, they have a cap 
and ration by queue or unavailability, etc. In the US we don't have a 
cap. 
      Anyway, with healthcare in the US, I defer to the great sage and 
noted philosopher Pogo: ":We have met the enemy and he is us."
