There'd have to be about 3 failures at the same time before it could go
off. Under the circumstances, I think it's as safe as any other.
As far as the challenge - a mechanical device which can't fail - how
about a rock? Technically, rocks, levers, inclined planes and so forth
count as mechanical devices. I suppose given a few thousand years one of
those would fail too, though.
By the same standards as the Colt, though, your weapon of choice
(whatever it may be) could also fail. Admittedly if it isn't cocked and
it fails, it probably won't go off - but I think failures which would
make it inoperable are more likely than the other kind on the Colt as
well.
And of course, brave is walking around unarmed because you're concerned
your gun might fail. I'm not that brave, sorry.
