No, in my 16 years as a Resp. therapist, I have never seen a regulator
sheared off.  Just heard about them though.  We did have a few tanks
explode on our fire department during a motor home fire last year.
Those did become airborne.  They were the smaller E tanks used for
personal use and when the motor home caught fire, they burst and sent
debris across the highway and started a brush fire on the other side.
That was a mess.  They are pretty tough when it comes to handling them.
