Was it a brand new tank? If so, did you treat the water?
Did the heater suddenly break in the "on" position?
Did you do a water change lately? Did you treat the water?
Did you add a lot of new fish to a tank that hadn't had many? What number of 
fish? What size of tank?
Your problem was probably one of the following:
a)      Chlorine or Chloramine in the water.
b)      Massive ammonia or nitrate spike due to increased bioload
c)      Massive Ph shift due to water change or increased bioload
