Once it succeeds, does it succeed every time?  I imagine that you put a
counter and loop in to keep trying until the open succeeded, but you may
have stopped at that point.  Modify your debugging code so that it keeps
trying even after it succeeds (closing first, of course) 10 times, and see
if it succeeds every time.  If it does, then you've certainly got a timing
problem.  There may be something wrong with the way you determine that the
external editor has terminated.  If it was a native NT application, for
example, the file may be held open by something invisible after the main
window has been closed.
