The top margin you can adjust just by putting empty paragraphs at the top of
the header (with a space in it so the system recognizes it).
The side margin you take care of by inserting an empty frame down the side of
the document in the header page. You might have to reset the top margin of
the document to be a negative number -- that part I don't remember -- in
order to force the letter text not to start on the next page. If the top
margin is a negative number, it acts as an "absolute" top margin -- it will
start where you tell it to, no matter what header text it might be
overprinting.
