Two choices I can think of.  One, create a Win95 DOS boot diskette (format a: 
/u /s) and boot from that floppy, then try reconfiguring the card.  If that 
doesn't work then the second option would be to pull the card from that 
machine and place it in another machine not running Win95 and attempt to 
configure it there.
If the above two suggestions don't work I'd be inclined to believe the NVRAM 
on the NIC has gone bad.  In that case, there's nothing you can do short of 
sending the card back to the manufacturer for refurbishment (most NIC's 
nowadays carry a lifetime warranty - you pay only shipping/handling).
