I had a friend who had a similar letterhead, and I set up a template for him 
that would handle this invisibly.  The trick was to set up an different first 
page header that had frames in the extra areas, so that text wouldn't go in 
those areas. 
  
The subsequent pages have a normal header (or no header), so they weren't 
affected by the first page setup.
