That's the equivalent of the VOX setting with InterCom, MANUAL is 
CB type and AUTOMATIC is the voice activated setting.
To call someone directly (just like with InterCom) they need to
be online not connected to the server and you call them at their
ISP address, with static addresses that's easy but most of us have
a dynamic address.  Though with InterCom you can logoff the server
once a connection to your party is established, keeps you from
getting buzzed while talking to someone.

Yes, the documentation is rather pitiful and I doubt that it will
improve, supposedly IBM pulled the plug on the OS/2 version.  The
price is right though (free) and it allows full duplex conversations
across different platforms if you are lucky enough to have the
right sound card.
