I've seen it.

I my case, it was a user's PC heavily infected with spyware.  I think it's
a dll hell type problem.  Anyway, cleaning out all the spyware fixed it.

I've also seen it where there was a big blank gap in the middle of that
list, so just scrolling further down got to the rest.  (I never did figure
that one out)
