I wouldn't know what the cause is in some organizations, say like some U.S.
News sites on the Web. Some sites at major T.V. Stations like NBCnews,
CBSnews, etc. often suffer the same problem you describe. I attribute it to
lazy editors or perhaps poorly educated, or efforts to get news out "fast"
(skip the editors). I can't know the reason though. Both Ron and I have
noticed it though, and it sticks out like a sore thumb when it happens. Very
noticeable and irritating.

Yes, I've had this happen. Surely aggravating :)
