I use either to just go directly from the old drive to the new drive, and
reset the partition sizes as I go. Backup to CD is good too, but since
with Ghost or Drive Image, the original drive is still intact, so there's
little chance of damage.
Another option, is get Partition Magic, and shrink the size of D, and
expand C. Shrink the size of the swap file on D first, reboot, then do the
resizing operation if you want to do this all on the original disk.
Personally, I'd use Ghost to clone the drive to a new larger drive, reboot
make sure everything is OK, and then upgrade. You'd still have the
original disk as a full fallback.
