AIUI, there are no major monitoring centers in the indian ocean... they're
in the atlantic and pacific... at least, that's pretty much what the report
stated that i heard a day or so after the incident...

as for who failed anyone? i understand that the cost of the monitoring
devices and getting them installed is pretty large and isn't as easily
performed as some imagine... the failure would be in getting the funds and
getting the devices installed... who is responsible for that? that's where
the failure occured...
