Yes, it could be an accident. My mom used to put our parakeets (in the
cage) on the patio on nice days because they liked to see and chirp at the
wild birds. I walked into the kitchen one day and stepped outside just in
time to stop the teal budgie Jeremy from flying out of the open cage door.
The green budgie had already taken off. Mom had left the cage door open
after having refilled the seed and water cups. To this day I feel bad I
yelled at my mother about it (I was about 15 and said some nasty things -
now that I'm almost the same age she was I certainly understand
forgetfulness).
However, having said all that, there is no reason to be leaving doors and
windows open or walking around outside with a bird who has full flight
feathers.
