Yep, had that happen once.  Only once though because I then had someone show 
me how check the rewind while advancing a new film to make sure things are 
working correctly.  It was very disappointing to end up losing that roll of 
pictures and I have been careful to check closely since then.  As usual, I had 
to learn the hard way though.
