Did you do any editing to the pppdial.cmd script?  If so, start out with a 
new unedited cmd file.
Did you include [PPPDIAL_DO_NOT_USE_DIALER]
OR ---          [PPPDIAL_USE_DIALER]          in your response file?
If you're using the dialer, you have to open MODIFY ENTRIES and add the 
response file to the LOGIN SEQUENCE.
*** [PPDIAL_MIN_CONNECT] ***   I don't recognize this command, either by 
spelling or function.
