The TSR (auto folder program) replaces the file selector
(fsel_input) with one with more features, including copy, move,
rename, and delete. The ACC simply calls fsel_input().

Perhaps I should have said that XBIOS has always supported bad
sector dectection, but I don't know if the GEM desktop uses those
features. However, every formatting program reads each track after
it is formatted and should at least fail during the formatting
process if a bad sector is discovered. Putting bad sector info into
the FAT is more complicated.
I really have no idea what your problem is, but you might try
formatting with something like UIS and see what happens.
BTW, have you cleaned your floppy drive lately?
