Better scanner or better antenna?
Scanner:  Radio Shack's Pro-43 handheld or Pro-2042 base/mobile.  The former 
is hard to find and the latter is expensive.  Still, out of all the scanners 
I've tried, they've been the best for me.  Your mileage may very.
Antenna:  If you're using the scanner in home or auto, just about any 
wide-band home or auto scanner antenna will increase sensitivity over the 
pack-in collapsable or rubber ducky antenna (though the deafness of duckies 
is greatly exaggerated).  I've had success using my car's radio antenna as a 
scanner antenna, thanks to some extra wire and an adapter plug.  Ditto for 
using a CB antenna as a scanner antenna.  Not as good as a "real" scanner 
antenna, but certainly good enough to do the job.  If you mostly listen to a 
single band, look for a HAM radio antenna 'tuned' to a frequency range near 
what you listen to.
If you aren't sufficiently confused by now, I can go on for hours if 
necessary.
