There is no 'standard' way to change pages -- what works on one chipset will 
probably fail on all others.
What you probably want is to look at Linear Frame Buffer of VESA 2.0. - your 
video ram appears as a continuous block of memory (1-8mb) at high address 
range. you then just use 32-bit addressing to access it. Very easy.

docs.
