The most likely scenario is the one you haven't listed: opening a link 
or an e-mail that carried the malware. Almost as likely: visiting an 
infected website. IOW, it doesn't matter where the infection took place, 
what matters is how the user granted access.

You may be able to generate a useful level of paranoia in the user.
