I'm currently using one which simply loads a binary image up to linear adress 
0x100000 (right above 1M), and jumps to it.

The info is available in the excellent book "The Indispensable PC Hardware 
Book", though it's nicely distributed across two chapters and an appendix..

Well, I switch quickly into it and back, to linearize the segments. This is, 
by the way, what FreeBSD does, I think.
