Super Mario Bros. was the first to use the standard formula.

They are, actually. In fact I was horrified when I found out 3D
Realms was considering making Duke Nukem Forever a side-scroller, and I
mailed Joe Siegler on the subject. Apparently many people also e-mailed 3D
Realms, because as you know, DNF will no longer be a side-scroller.
